UnitedHealth Group (UNH) reported revenue of $112,032m for the second quarter of 2026, almost unchanged from a year earlier, and net earnings of $5,484m, up 61%. The medical-costs-payable note of the 10-Q filed on August 10 shows where the rebound came from: a release of reserves set aside for last year's claims.
What happened
UnitedHealth Group (UNH) is the largest U.S. health insurer and operates Optum (care delivery, pharmacy, data).
Its Q2 2026 quarterly report (10-Q) shows revenue of $112,032m, roughly flat against $111,616m a year earlier, while earnings from operations rose 55% from $5,150m to $7,991m and net earnings attributable to shareholders reached $5,484m (+61%). The medical care ratio (MCR) fell 2.7 points from 89.4% to 86.7%.
The medical-costs-payable rollforward in Note 4 splits first-half medical costs of $148,847m into two lines: $150,333m incurred in the current year, and −$1,250m for prior years — reserves set aside for last year's claims that proved larger than needed and were released from this year's costs. The same line in the first half of 2025 was −$320m: 3.9 times smaller. A further −$236m comes from changes in premium-deficiency and loss-contract reserves.

No. Adjusting estimates to actual experience is normal, and the 10-Q discloses the amount and reason (a favorable respiratory season). But the release is 3.9 times a year earlier and is a revision of last year's estimate rather than this year's operations, so it belongs to the composition of earnings. The criteria for judging it are in the subscriber section.
What are the government investigations?
The 10-Q says the company is involved in investigations, audits and reviews by agencies including CMS, the DOJ, the SEC, the FTC and Congressional committees, is responding to subpoenas and information requests, and that certain businesses are under review including for compliance with coding. It says it can give no assurance on scope, outcome or material effect. This article reports only what is disclosed.
How did net earnings rise 61% on flat revenue?
Medical costs fell $3.2bn from a year earlier ($78,585m → $75,358m) and the MCR fell 2.7 points, lifting operating earnings from $5,150m to $7,991m. On a first-half basis, the $1,250m of prior-year development and the loss-contract reserve reduction are part of that decline.
